When you go on an event sponsored by Crux Student Ministries, you need to fill out the Insurance Form and the Release Form that lists all of your important information. Please download and print these forms, fill them out, and bring them to Pastor Devin with your payment for the event, so that you can go with us.
If you don't have these forms on file,
you will not be able to attend the event.
( amusement park trips, retreats, camps, etc. )
